The role:

The Outpatient Clinic Nurse serves as a member of a multidisciplinary team to assess, plan, develop, coordinate and provide treatment, rehabilitation, and support services to a telehealth team in an outpatient program.

The RN/LPN supports the prescribing professionals/patient/agency with medication management and referrals as necessary for additional services. RN/LPN functions as a clinical member of the multidisciplinary team to provide nursing assessment and care, oversight of medications, support, and other rehabilitative services to the patients.

What you will need:
  • A current RN or LPN license is required.
  • Knowledge and work experience with mental health and child welfare is required.
What you will do:
  • Use available data, such as assessments, participant evaluations, and service observations to identify health and wellness needs to develop individualized care coordination plans.
  • Organize and manage the system of getting medication to patients and integrate medication into patient’s individual treatment plan.
  • Support ongoing collaboration between physical health and behavioral health providers.
  • Must coordinate and perform injections as needed for patients as necessary.
  • Responsible for managing and monitoring the refrigerated medications and complying to regulations on refrigerated medication.
  • Provide health education to clients on preventing and managing chronic conditions.
  • Monitor high-risk patient cases and facilitate communication with prescribing professional as necessary.
  • Schedule and coordinate initial, crisis, intake and other appointments with clients.

Compensation:

LPN: $21-22/hour

RN:$24-25/hour
