YSS is seeking a full-time Care Specialist to join the Integrated Health Services (IHS) team at our Ames Family Life Center location.
The IHS Care Specialist serves as a member of the Integrated Health Services team to coordinate services for mental/physical health and provide support to families. This position develops care plans with the family, completes  assessments, makes and tracks referrals, and helps them connect with community and professional resources. The Care Specialist collaborates with families and their existing providers, including physicians, therapists, schools and/or social service agencies.
Qualifications:
  • Bachelor’s degree in a social work or related human services field is required.
  • Knowledge and work experience with mental health and child welfare is required. Work experience may be substituted for the educational component.
Responsibilities:
  1. Use available data, such as assessments, participant evaluations, and service observations to identify health and wellness needs to develop individualized care coordination plans.
  2. Assess for informal and formal support systems
  3. Facilitate individualized care planning and mutual goal setting.
  4. Educate clients and their families or care advocates in wellness, medication and behavioral health services.
  5. Complete required written reports and documentation according to policy.
  6. Develop crisis plans as necessary.
  7. Provide transportation to appointment when necessary.
  8. Coordinate and be familiar with all aspects of outpatient funding, including understanding of EMR and funding coordination internally as well as credentialing and compliance with third party payers.
  9. Schedule and coordinate initial, crisis, intake and other appointments with clients.
  10. Demonstrate flexibility in job duties and schedule in order to serve clients and to assist reaching benchmark standards.
  11. Participate in ongoing quality improvement efforts and monitor/evaluate service outcomes.
  12. Attend regularly scheduled meetings and training sessions to enhance team performance.
  13. Participate in various workgroup and committee activities to support efforts to improve the quality of services.
  14. Assist in special projects as assigned by supervisor.
